In-Home Assessment & Personalized Care Planning


Every family's situation is different. We meet with your loved one in their home to understand their needs, identify concerns, and create a thoughtful plan focused on safety, independence, well-being, and quality of life.

Care Coordination & Trusted Resources


Finding the right care shouldn't feel overwhelming. We help coordinate home care, medical services, transportation, senior resources, and other trusted professionals—providing one knowledgeable point of contact to help keep care organized.

Family Guidance, Advocacy & Ongoing Support


When you can't be there for everything, we can help. From navigating difficult decisions to attending appointments and monitoring changing needs, we provide families with trusted local guidance, advocacy, and peace of mind.

Plans

  • PERSONALIZED CARE — Pay As You Go

    $200/hour

    Professional guidance when you need it, without a monthly commitment. Ideal for families who need help navigating a specific concern, finding resources, or making an important care decision.

    Includes:

    • In-home or virtual care-management consultations

    • Personalized care recommendations

    • Assistance locating home care, transportation, senior programs and community resources

    • Coordination with providers and services

    • Family consultations and care-planning meetings

    • Advocacy and assistance navigating changes in care

    • Medical appointment accompaniment when requested

    • Phone and email coordination

    New clients begin with a Comprehensive In-Home Assessment & Care Plan — $550.

  • PEACE OF MIND — Monthly Care Management

    $795/month

    For families who want an experienced professional consistently involved in their loved one’s care. We get to know your loved one, stay connected, and help address concerns before they become overwhelming.

    Includes up to 4 hours of care-management services each month:

    • Monthly in-home wellness and needs visit

    • Personalized care plan with ongoing updates

    • Coordination with home-care agencies, medical providers and community services

    • Family communication and monthly care update

    • Assistance coordinating appointments, transportation and services

    • Advocacy and problem solving when concerns arise

    • Guidance with changing care needs

    • Phone and email support

    • Priority scheduling

    Additional care-management time: $200/hour.

  • CONCIERGE CARE — Comprehensive Monthly Support

    $1,495/month

    Our highest level of support for families who want a trusted professional closely involved in their loved one’s care. Especially helpful for older adults with complex needs, multiple providers, changing health or living situations, or family members who live at a distance.

    Includes up to 8 hours of personalized care-management services each month:

    • Up to two scheduled in-home visits per month

    • Comprehensive and continuously updated care plan

    • Regular monitoring of changing needs, safety and available supports

    • Coordination across home care, medical providers, rehabilitation and community services

    • Medical appointment accompaniment and advocacy as needed

    • Assistance navigating hospital-to-home and rehabilitation transitions

    • Senior-living and higher-level-of-care guidance

    • Family meetings and decision-making support

    • Regular family updates

    • Coordination of transportation, appointments and supportive services

    • Collaboration with elder-law attorneys and other professionals when appropriate

    • Priority phone and email support

    • Priority scheduling for emerging concerns

    Additional care-management time: $200/hour.

Care-management services are non-medical and do not replace emergency services, medical care, home-health services or psychotherapy. Monthly services are subject to the terms of the care-management agreement. Unused hours do not roll over.
